ASQ Alternate Information Summary Sheets

ASQ Information Summary sheets are now available in an alternate format. These alternate sheets use a scoring chart design that does not incorporate shading to differentiate results.

Programs who have purchased ASQ-3 in English may complete this form for free access to the alternate files. Files for ASQ-3 in Spanish and ASQ:SE-2 in English and Spanish will be available later this fall.

To access the Alternate Information Summary sheets, complete the Request Form below and agree to the terms of use. After submitting the form, you will be able to download a zipped folder of files.
